Your Provider

Riana Leonhardt, MSN, FNP-BC

I’m a Board-Certified Family Nurse Practitioner with almost ten years of experience. I take an integrative and functional approach, looking at the whole person and building a real connection with my patients. I believe everyone is unique, with lots of different factors that affect their health. I love working closely with my patients to get to the root of their symptoms and create personalized plans that focus on natural healing whenever we can.

Founder and Integrative Health Care Provider, Lily Sage Health

I believe everyone is the expert in their own healing. My role is to be a trusted partner—supporting, guiding, and empowering you to reach your health goals. Real health happens mostly outside the doctor’s office, in your daily life. That’s why my approach is rooted in the power of self-love and self-care as the foundation for lasting wellness.

I believe that each Individual has the leading role in her health story. I want to empower my patients to realize that they can take their health and healing into their own hands.

When I’m not working, you’ll probably find me experimenting with healthy recipes, exploring new trails on a hike or walk, tending my garden, or hanging out with my husband, kids, and our dog. I’m a lifelong learner with a growing stack of books overdue at the library. Music is a big part of my life—don’t be surprised if you catch me dancing around the living room with my kids!

fun facts

At Lily Sage Health, my approach combines integrative and functional medicine with a strong foundation in conventional care. I use the best available evidence to create personalized treatment plans that reflect each person's unique needs.

I take the time to understand your full health story and work as a partner in your care, always focused on uncovering the root causes of symptoms and illness. I view each patient as a whole person, with many interconnected factors shaping their health.

My approach is thoughtful and conservative. I lean toward natural, lifestyle-based interventions whenever appropriate, but I also value conventional tools like medication or specialist referrals when needed. True healing often happens through the right balance of both.

My Approach: Integrative and Functional Medicine grounded in the latest evidence
My approach blends the principles of functional and integrative medicine, with a focus on treating the whole person rather than just  symptoms. I work to uncover the root causes of illness by considering each individual's unique biology, history, lifestyle, and environment. Health, to me, is a dynamic state of resilience and well-being, not just the absence of disease.

Grounded in science and informed by a systems-based perspective, I explore how life events from preconception to the present may contribute to current health issues. I aim to support the body’s natural healing ability with targeted, effective, and minimally invasive strategies tailored to each person.

I believe healing is a collaborative process. I value the relationship between patient and provider and encourage active participation in each step of the journey. I integrate conventional medical care with complementary therapies that are critically evaluated for safety and effectiveness. Each plan is designed to support the full spectrum of health—mind, body, spirit, and community.

Prevention and wellness are at the heart of my practice. By addressing the underlying causes of imbalance and supporting optimal function, I help patients move toward lasting health and vitality.

step two

After choosing your wellness plan, you’ll complete our integrative health intake analysis and schedule and complete your first appointment. The first appointment is a 120-minute ‘deep-dive’ into your health history, and no detail is too specific. I will order comprehensive lab tests for you to complete prior to your second appointment, and we will begin to construct your personalized plan to achieve optimal health and wellbeing.
