Your Provider

Riana Leonhardt, MSN, FNP-BC

I am a Board-Certified Family Nurse Practitioner with the American Nurses Credentialing Center. I have nearly a decade of experience working with patients as a Nurse Practitioner. I have an integrative and functional medicine approach that prioritizes focusing on the whole person, my relationship with my patients, and identifying the root causes of symptoms and disease. I believe that each individual is a complex being with various factors that influence the experience of health and well-being. I am passionate about working closely with my patients to discover the root causes of symptoms and developing a personalized plan that emphasizes natural approaches to healing when possible.

Founder and Integrative Health Care Provider, Lily Sage Health

Each individual is an expert in her healing ability. I want to be an active partner in my patients healthcare journey by supporting, guiding, and assisting you in reaching your health goals. I understand that true health primarily occurs in what happens when you are not in your health practitioner’s office. Finally, my approach to health is grounded in the belief that self-love and self-care are necessary foundations in ones health journey.

I believe that each Individual has the leading role in her health story. I want to empower my patients to realize that they can take their health and healing into their own hands.

You will likely find me cooking and trying new healthy recipes, out walking or hiking, gardening, or hanging out with my husband, children, and our dog in my free time. I am a lifelong learner with an active pile of books needing to be returned to the library. I love music and you just might find me having a dance party in the living room with my kids.

fun facts

My approach at Lily Sage Health is a blend of integrative and functional medicine with a foundation in conventional medicine. I use the best evidence available to tailor treatment plans to each individual. I act as a partner in health and take a deep dive into my patients history in order to determine the root causes of symptoms and illness. I always look at each patient as a complex being with multiple aspects contributing to their health. I take a conservative approach and prefer to utilize natural interventions as first line when appropriate. That does not mean that I would not prescribe a pharmaceutical or place a referral to a specialist when it is needed. There is room for the interplay of these approaches on the journey to health.

credentials

Important principles of integrative medicine include:
  • The patient and health provider act as partners in the patients healing journey.
  • Conventional and alternative healing methods are used in conjunction to promote healing.
  • The least invasive interventions are used first when it is appropriate and possible.
  • All the factors that influence one’s life are considered in the healing journey including lifestyle, mind, body, spirit, larger environment, and community.
  • Grounded in science and evidence, all therapies and interventions are evaluated critically.
  • Prevention of disease and promotion of health and wellness are prioritized.

What is Integrative Medicine?

Important principles of functional medicine include:
  •  Functional medicine is grounded in biology and science and is focused on identifying the root causes of illness, all while treating the individual and not the disease.
  •  Treatment approaches that address the root causes are emphasized.
  •  Events in one’s entire lifespan from preconception to present day are taken into consideration and how these events may be contributing to current health are explored.
  •  Functional medicine stresses that the body has an innate ability to heal when it is fully supported

next step

step one

If you would like to take the next step in your health journey towards feeling better and healing, start by scheduling a free 20-minute Discovery Call with me to learn more and see if our process and approach are a good fit for your unique health situation.
I will learn more about your symptoms and goals; you will learn more about me and my approach at Lily Sage Health. We will then decide on the Wellness Plan that best suites your needs and schedule your first appointment.

next step

step two

After choosing your wellness plan, you’ll complete our integrative health intake analysis and schedule and complete your first appointment. The first appointment is a 120-minute ‘deep-dive’ into your health history, and no detail is too specific. I will order comprehensive lab tests for you to complete prior to your second appointment, and we will begin to construct your personalized plan to achieve optimal health and wellbeing.
